Daily tasks can become challenging as we age, but seeking help does not mean losing your independence or the comfort of your home. The Commonwealth Home Support Programme (CHSP) is designed to assist older Australians by providing entry-level support services so that they can live independently and safely in their own homes.

Is CHSP Right for You?

The CHSP is ideal if you find everyday tasks difficult and believe a little support could significantly improve your health and well-being. You are eligible for this program if you are:
  • 65 years or older, or 50 years or older for Aboriginal or Torres Strait Islander people.
  • 50 years or older and on a low income, homeless, or at risk of being homeless (45 years or older for Aboriginal and Torres Strait Islander people).
  • An aged care assessment will determine your specific service eligibility.

How to Get an Assessment

To be eligible for an assessment, you must meet specific criteria:
  • Use our eligibility checker tool to answer a few questions.
  • If eligible, you can apply for an evaluation directly.
  • Once approved, an assessor will arrange to meet you at your home to discuss your needs.

The Kanyini Care Memory Kit is designed to improve the well-being of our clients through engaging and stimulating activities. These activities are crafted to enhance cognition and include puzzles, memory card games, sensory and creative activities, reminiscence prompts, and musical therapy guides. The kits are available to all clients living with dementia or experiencing cognitive decline who are receiving social support or respite services from Kanyini Care.

Meals and Food Preparation:

Ensuring you continue to eat well.

Bathing, Hygiene, and Grooming:

Maintaining personal hygiene standards.

Nursing:

Treating and monitoring medical conditions at home.

Therapies:

Including podiatry, physiotherapy, and others to maintain mobility.

Impairment and Continence Help:

Managing conditions to maximise independence.

Respite Care:

Offering day/overnight breaks for you and your carer.

Home Modifications:

Enhancing your home’s safety and accessibility.

Home and Garden Maintenance:

Keeping your living environment safe.

Cleaning and Laundry Services:

Helping maintain a clean and livable home.

Aids for Independence:

Supporting mobility, communication, and personal care.

Transport Services:

For appointments and community activities.

Social Outings and Activities:

Encouraging community interaction.

Homelessness Support

We also provide support for older individuals who are homeless or at risk of homelessness, including access to housing and care services.

Cost of Services

While services are subsidised, if you are eligible, you are expected to contribute to the costs based on your financial ability. Costs vary by service type and provider, and discussing these with your provider directly is essential.
